The Padrón Municipal de Habitantes is Spain's official municipal census. Every person who lives in a Spanish municipality ~ Spanish citizen or foreign national ~ is legally required to register at their local ayuntamiento (town hall). In Madrid, this is managed by the Oficinas de Atención Ciudadana (OAC).
Registering doesn't give you residency rights or replace your TIE card. What it does is create an official record that you live at a specific address in Madrid ~ and that record is the key to unlocking almost everything else in the Spanish administrative system.
The empadronamiento certificate (volante de empadronamiento) is the printed extract you'll use in appointments. Most institutions require one that's less than 3 months old ~ so you'll request a fresh one before each major step.

Required in Madrid before you can book your TIE cita previa at the Oficina de Extranjería. Must be less than 3 months old.
Your Centro de Salud requires a Padrón certificate to assign you a doctor and issue your health card.
Children must be registered at the address in their catchment area. Required for public and subsidised private schools.
Santander, BBVA, and CaixaBank often require a Padrón certificate for foreign residents opening an account.
UGE requires a Padrón certificate issued within the last 3 months as part of your Digital Nomad Visa renewal pack.
Madrid library card, EMT transit discounts, municipal gym memberships, and local subsidy applications all require it.

Register 24/7 through madrid.es using a digital certificate (Cl@ve PIN, DNIe, or FNMT certificate). If you already have a digital identity in Spain, this is by far the quickest route.

In-person registration
If registering in person, book a cita previa at sede.madrid.es by searching 'Alta Padrón Municipal' ~ or call 010. Appointments in central districts can be 1-3 weeks out in September and January. If you have Cl@ve PIN or a digital certificate, skip the appointment and register directly online.
The Hoja de Inscripción en el Padrón Municipal is available at any OAC or downloadable from madrid.es. Fill in your full name, date of birth, nationality, NIE/passport number, and the exact address you'll be registering at. Each person in the household needs their own form.
Bring your passport or NIE/TIE (original + photocopy), rental contract (at least 6 months, in your name or with the landlord's authorisation letter), and proof of rent payment (recent bank transfer screenshot is fine). Also bring your completed Hoja de Inscripción.
Arrive on time ~ the OAC system is strict. The official will verify your documents, enter your details into the municipal register, and stamp your Hoja de Inscripción. If everything is in order, some offices will issue your certificate immediately.
The volante de empadronamiento is the document you'll actually use. You can request it on the day, or later online at sede.madrid.es. It's free and there's no limit to how many copies you can get ~ print several because you'll use this for your TIE, bank account, healthcare registration, and DNV renewal.

Scanned or photographed reverse side of your National Identity Card, Passport, or Residence Card (NIE/TIE).

For guardianship, custody, foster care, or adoption cases ~ required to register a minor under your care.
Declaration of residence
An additional supporting document sometimes requested in unusual address situations.

After registration
Request your first volante de empadronamiento the same day at the OAC, or online via Sede Electrónica once your registration is confirmed. Print multiple copies ~ you'll need them for your TIE, bank account, and healthcare registration.
The certificate expires after 3 months for official use. Renewing is instant ~ request a fresh copy online at sede.madrid.es with Cl@ve PIN, in person at any OAC, or via the 010 phone line. Your registration never expires.
Common Questions
Technically yes, but your friend must provide a signed authorisation letter, a copy of their DNI/NIE, and proof that they live at that address (their own empadronamiento or rental contract). The OAC will verify. This is common when you first arrive and are staying temporarily.
If you register in person and the office is not busy, you can get the volante de empadronamiento on the same day. If the certificate is posted, expect 1-3 weeks. Once registered, you can always request a new certificate immediately online if you have a digital certificate (Cl@ve).
The registration itself does not expire ~ but the certificate (volante) is only valid for 3 months for official use (TIE appointments, DNV renewal, bank accounts). You'll need to request a fresh one each time. Requests are free and instant if done online.
Yes ~ you have 2 months to update your Padrón when you move. Failure to update can cause problems with your TIE renewal and healthcare assignment. Simply book a new OAC appointment and bring your new rental contract.
Yes. The Padrón does not require you to have residency status ~ it only requires proof that you live at the address. Many people register while awaiting their DNV approval and use the certificate to speed up their TIE process once the visa comes through.
Each person must complete their own Hoja de Inscripción, but a family can register together at the same appointment. Children under 16 are registered jointly with their parent or guardian.
